Please COMPLETE the following checklist prior to our first skate on Wednesday, September 2, 2009
1.Go to
http://www.usahockeyregistration.com/ and register your child ($35 for kids six and older). Don't lose your IMR number - you will need this to register at grosseilehockey.com!
2. Go to http://www.grosseilehockey.com/index.php and register your skater - the cost is $100 3. Make a COPY of your child's birth certificate, and bring it to the first skate on 9/2/09 4. Print the following MAHA forms and bring them to the first skate on 9/2/09 http://www.maha.org/Handlers/File.ashx?FileId=1422 http://www.maha.org/Handlers/File.ashx?FileId=58 http://www.maha.org/Handlers/File.ashx?FileId=777 5. PARENTS - if you will be joining your child on the ice, please register at USA Hockey for an IMR. The cost is $35 (see step one of the checklist). Print your receipt with your IMR and bring it to the first skate on 9/2/09 6. PARENTS - complete a background check at http://www.maha.org/CoachingBackgroundCheck.aspx Print your email confirmation and bring it to the first skate on 9/2/09 7. KIDS - Full hockey gear is required to skate - Visit Perani's and/or Play It Again Sports in Taylor to purchase equipment PARENTS - Helmets are required on the ice
8. There will be a small fee for your child's jersey - more details coming soon!
